Authorization and Release Information

The undersigned person agrees as follows:
1. I understand my testimonial, as outlined below, may be used in connection with publicizing and promoting Usbergo, Inc. doing business as “EcoProProducts.”
2. I authorize the use of my name, business name, photo, and likeness, together with my endorsement or testimonial, whether written or oral, which may appear in any media released by EcoProProducts in connection with any advertisement or promotion of EcoProProducts appearance protection products and/or services including, but not limited to, brochures, Usbergo, Inc.’s or American Guardian Warranty Services, Inc.’s (“AGWS”) website, and any and all social media sites.
3. I understand that my name will be disclosed in conjunction with my testimonial or endorsement.
4. I agree that EcoProProducts may copyright my endorsement or testimonial in its name or any other name it may choose.
5. I authorize EcoProProducts, its agents, representatives, and assignees to use, reproduce, exhibit, and distribute any promotional materials containing my name, business name, and photo, together with my endorsement or testimonial, for the purpose of endorsing its products and/or services.
6. I hereby certify that my endorsement or testimonial of EcoProProducts’ appearance protection products and/or services is true and accurate and reflects my personal experience and honest belief in the quality of the products and/or services that I am endorsing or providing a testimonial. I am doing this testimonial of my own free will and I have not been paid a monetary sum for doing so.
7. I hereby certify that I am over twenty-one years of age and competent to contract in my own name.
8. I understand that this Testimonial Release Form may not be amended without my written consent and the written consent of EcoProProducts.
9. I have read and fully understand this Testimonial Release Form and I am signing it voluntarily.
10. I hereby release EcoProProducts, Usbergo, AGWS, its officers, employees, and agents from any and all claims and damages arising out of or in connection with use of my testimonial.
